Job Description
Job Title: Program Instructor
Department: Day Services
Reports To: Day Program Manager
SUMMARY
A Program Instructor provide day support service which is necessary to support persons with disabilities to participate in a day program and drive a minibus, van or lightweight truck to transport individuals, trainees, or company personnel. This job requires transportation training and knowledge of Activities of Daily Living (ADLs) responsibilities.
ESSENTIAL DUTIES AND RESPONSIBLITIES
Responsible for direct daily care needs of individuals, ADLs and recreational activities, and assistance in aiding individual learning and development.
Supports in planning and executing daily activities of individuals with other staff, Program Leader and Managers.
Accompanies and transport individuals to participate in recreational and community outings.
Provide training based on Individual Support Plan (ISP); follows ISP as developed; serve as goal manager and document an individuals achievement toward acquisition of developmental goals.
Responsible for transferring into and out of wheelchair and motor vehicle; use of Hoyer Lift as needed.
Drive vehicle from individual or central loading area to social services or rehabilitation center, training location, job site, or other destinations according to assigned schedule.
Maintain a safe driving record and comply with all local traffic regulations.
Keep accurate daily documentation of activities, medications, ISP tracking, as well as other documentation as required.
Adhere to emergency preparedness and response procedures, ensuring the safety of the individuals; Reports all individuals incidents to supervisor immediately and follow policy regarding reporting such incidences.
Observes and report any changes in circumstances that could have a detrimental effect on the safety of the individuals (including the physical environment), any changes in condition or any complaints about care to the Lead Staff, Program Manager and/or Director of Community Life.
Understand and respects the right of each individual, maintains confidentiality of each individual, encourages individuals to exercise their rights, enhances personal decision making, promotes self-esteem and ensures that the protection of the individuals from abuse and neglect.
Communicates professionally and treat with respect the people we support, families, service coordinators, and InCommunity employees.
Demonstrates and perform as an effective adult model for the people we serve.
Maintain supervision of the individuals we support, unless it is stated in the Plan of Care.
Consult with all staff on progress and/or problems
Remain on duty for the hours assigned and implement planned schedule.
Obtain 16 hours of training which is task focused on related job assignment.
Perform all other duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

EDUCATION/EXPERIENCE
High School diploma and six months experience in working in human services or related field preferred. Personal and volunteer experience considered for employment.
